TERMS AND CONDITIONS OF MEMBERSHIP
Upon becoming a member of 360 Health & Fitness you agree to abide by the following terms and conditions.
FACILITY RULES
All members are required to follow all the rules of the facility in which Bootcamp and Personal Training sessions are being held, including, but not limited to, sign in procedures, appropriate workout attire, and appropriate behaviour standards.
Failure to abide by any of the rules of the facility or any other requests at the reasonable discretion of a staff member of the facility, members will be asked to leave. Members found to be repeat offenders may have their memberships suspended or cancelled. Management will not be held responsible for any accident of injury suffered, or any loss or damage sustained to members or their personal property while using the facilities.

COMPLETE AND ACCURATE DISCLOSURE:
I agree that all information given by me to 360 Health & Fitness is truthful, complete and accurate. I also agree to answer any questions asked by 360 Health & Fitness that 360 Health & Fitness deems necessary in relation to my use of the facilities.
INVALID CLAUSES:
If any part of this Agreement is held to be invalid, unenforceable or illegal for any reason, this Agreement will be deemed to be amended by the addition or deletion of wording necessary to remove the invalid, unenforceable or illegal part, but otherwise to retain the provisions of this Agreement

MEDICAL AND HEALTH ACKNOWLEDGEMENTS AND CONSENTS:
- I confirm that to the best of my knowledge I suffer from no physical, medical or mental condition which may be significantly aggravated by my proper and reasonable use of the facilities. Any conditions I may suffer from have been disclosed to my trainer prior to commencing my first session.
- I acknowledge that I have been advised to consult a doctor before taking part in any exercise programme, and that I will follow any instructions or advice given to me by them.
- I consent to receive medical treatment in the case of injury or illness which I may suffer connected with my use of the facilities and indemnify 360 Health & Fitness (and its contractors) to the extent permitted at law against any claims or expenses howsoever in respect of such treatment.
MEMBERSHIP FEES:
Subscription Fees
- I agree to pay the sum as outlined on my membership form for the services signed up for on a weekly basis, unless another arrangement has been entered into, in writing, with the management of 360 Health & Fitness.
- 360 Health & Fitness reserves the right to charge a $10.00 accounts management fee in the event that a weekly payment is missed.
- Any adhoc or extra sessions outside of my membership arrangement must be paid within 5 working days of the session.
- Invoices will be provided on request only.
SUSPENSION AND/OR CANCELLATION OF MEMBERSHIP:
Membership Suspensions (Holds)
- I acknowledge that I may put my membership on hold from 2 to 8 weeks, with 1 weeks notice in writing to [email protected]. Failure to do so will result in normal fees continuing to be charged and payment will be required to be made.
- Membership Cancellation
- I agree that to cancel my membership, I must give 28 days notice in writing to [email protected], and that all membership fees are to be paid up until the 28th day of notice.
- Any membership fees paid in advance at date of termination of this agreement will be refunded within 28 days of the termination date.
SESSION CANCELLATION/CHANGE POLICY
Cancellation/Change by Member
- Any cancellations or changes to session times made by members for personal training sessions are required to give a minimum of 24 hours notice. Failure to provide the correct notice will result in a fee of 100% of the agreed session price being charged to the member.
- If a member fails to show for a session or cancels their session within twenty four hours of the session start time, a charge of 100% of the session cost will be charged.
- This clause applies to individuals and groups that are booked in for a session.
- If a member is unable to attend a general Bootcamp session, no notice is required unless it was the boxing session that requires prior registration.
- Any cancellations or changes to session times made by trainers will be with a minimum of 24 hours notice, where possible, and/or a substitute trainer may be provided.
